Centering Your Healthcare
Your personal provider and an extended team of health professionals, build a relationship in which they get to know you, your family circumstances, your medical history and health issues. In turn, you can come to trust and rely on them for expert health care answers that are suited entirely to you or to your family. Caring about you is the most important job of your Patient Centered Medical Home. In this personal model of health care, your primary provider at Alliance Community Healthcare leads the team of professionals that take responsibility for your care. We make sure you get the care you need in wellness and illness.
Medical Home Responsibilities
Patient Centered care at Alliance Community Healthcare involves collaborating with your provider to develop a personalized care plan that not only addresses your immediate health concerns but also focuses on maintaining your overall well-being, providing support whenever you need it.
At Alliance Community Healthcare:
We will ensure that your care is coordinated across all elements of healthcare system.
We will advocate for you to get you the care you need.
We will provide enhanced access to care with expanded hours, open scheduling, and options for communication with your team.
We will maximize information technology.
We will measure, track and aim to improve your health performance.
We will facilitate the referral process.
We will provide results of lab/x-ray tests by calling the patient/parent.
We will make clinical decisions utilizing evidence based medicine and clinical decision support tools.
We will participate in electronic prescription programs with affiliated pharmacies.
We will respect the patient/parent individuality and be culturally/ethnically sensitive.
Patient Responsibilities
Come prepared for your appointments: understand that you are a full partner in your own health care.
Learn about your condition and what you can do to stay as healthy as possible. Be honest about your history of symptoms.
As best as you can, follow the care plan that you and your medical team have agreed is important for your health.
Bring a list of any medicines, vitamins or remedies you use and update us on any new medications or pharmacy changes.
If you don’t understand something your provider or other member of your team says, ask them to explain it a different way.
If you get care from other health professionals, always tell your medical home team, so they can help coordinate for the best care possible.
The patient/parent will pay for their share of the provider services rendered not covered by their insurance at the time of the office visit. It is the patient/parent responsibility to know their insurance benefits.
The patient/parent will keep their appointments; otherwise, a missed appointment fee may be applied.
The patient/parent shall arrive on time for their scheduled appointment.
The patient/parent shall follow up with any referrals made by our team and under- stand the insurance coverage for the specific referral.
