Insurances We Accept
Currently, Alliance Community Healthcare accepts most insurances, we have listed some of them below.
This information is subject to change at any time, please confirm if your insurance is accepted at the time you schedule your appointment. If you do not see your insurance listed, please give us a call to inquire if it is accepted.
What is the Sliding Fee Scale?
As a federally funded Community Health Center, we are in a position to offer medical, behavioral health, specialty and dental services on a sliding fee scale. This means that the portion you pay for your healthcare will depend on your family size and income, as determined by our Sliding Fee Scale Application.
The health center has established a medical nominal fee of $25.00 and dental nominal fee of $40.00 at time of service. If you are unable to pay at the time of service, a Payment Plan can be established with an initial deposit and the balance at an affordable pace.
For patients with a balance, the Payment Plan is also an option that will allow you to remain in good standing with the health center.
Please remember – without your continued support, we will be unable to provide this much-needed service for our community, please do your part by Paying your Bill! We offer online payment options for your convenience, or you can stop by any of our locations and make your payment.

Uncompensated Care Program
Based on a similar criteria, we can also assess you for our Uncompensated Care Program. This program allows us to offer primary medical, dental care and mental health services for uninsured New Jersey residents with family incomes at or below 250% of the Federal Poverty Level.
A New Jersey Resident, defined as a person who maintains a permanent customary home in the State, with the intention to remain in the State, and the home is not established for a temporary purpose such as for a visit or vacation. A resident may live in his or her own home, a rented home or apartment, the home of a friend or relative, in a residential health care facility, boarding home, homeless/domestic shelter, in a long-term care facility, or other acceptable alternate living arrangement. In order to participate in this program, an application would be completed for eligibility assessments as well as backup documentation.
