Welcome to our new website
On behalf of our Board, My Team and myself, I Thank you for visiting us. Alliance Community Healthcare has been serving Hudson County since 1963. I am enthusiastic about our future, despite several challenges along the way. I have always believed deeply that healthcare is everyone’s right and access to high quality healthcare care is our mission.
Long before social determinants of health became a buzz word in healthcare, Alliance focused on addressing those issues, working to prevent, substance abuse, homelessness, sex trafficking, domestic abuse, mental illness, homelessness, and numerous other issues that impacts health and healing.
Our history speaks for itself, starting as a Family Planning program, adding social services, and some Internal medicines services as the need became evident, and eventually adding comprehensive Primary Healthcare services and Dental to meet the needs of our patients. A few name changes over the years, from Horizon Health Center, to Alliance Community Healthcare, with a focus on community, service and the highest standards in care, Alliance has been and continues to be an accredited Federally Qualified Health Center (FQHC), Joint Commission Accredited and NCQA Recognized.
The transition to becoming an FQHC, was our way of committing to addressing the needs of the community. Our mission from the start was to provide access to healthcare, offering Sliding Fee Scale Discounts and enabling Social Services onsite as an integrated component in your healthcare. That holistic approach to care delivery, allowed us to address and support the Mental, Behavioral Health and Wellbeing of our patients in their Medical Home, boosting their chances for improved health outcomes.
Alliance services are comprehensive, with an advance Electronic Healthcare (Medical and Dental) Record, Alliance’s Doctors, Dentist and any other Healthcare Provider or Health System you may have reason to engage can quickly, and effectively communicate on your health status and thereby work together to avoid costly medical errors. As your Healthcare Home we provide you a central repository of your Healthcare record, accessible to you 24/7 via your Alliance Patient Portal or Mobile App. Whether you need urgent care or to request a medication refill, you can control, manage or share your health information as needed. Our integrated model of care works to eliminates gaps in care issues, with the support of your Alliance Care Team, inclusive of our Dental, Behavioral Health and Community Services Teams, we are here when you need us, your one stop Point of Care service. Ready to be your Ally in health and life.
Our Commitment is to provide you with the best healthcare service on demand. Our promise to you is Continuous Quality Improvement (CQI), in the care we provide as expressed by our continued certifications from Joint Commission, NCQA and HRSA Quality Leader and FTCA continually deemed. We continue to use technology to analyze performance and strive to continue to surpass our clinical goals. When we fall short, we step up and act using your feedback and our Continuous Performance Improvement plans, to address and resolve any issues. We love to hear from our patients in person, via your patient portal or through our patient surveys, we work to get better every day to earn your trust in us as partners in your healthcare and we continue to serve you and support you to be in your best health, to manage and control any chronic health conditions you may have been diagnosed with to live well and not be defined by a diagnosis, we will continue to work to meet your needs and expectations and then exceed them.
