What to Expect
As long as you are a patient with Alliance, you will be cared for by a primary care physician of your choosing. Typically, your primary care physician will act as the first point of entry into your health care services and will oversee all aspects of your care.
Your primary care physician will also be there to diagnose of any illness or health issue (except major emergencies), and provide follow-up care.
Most patients find that mutual understanding between patient and primary care physician grows over time, so he or she is likely to continue to be your “go to” provider for most of your experience at Alliance.

Healthcare tailored for you
Your primary care physician is specially trained and skilled in comprehensive first contact and continuing care for chronic health issues such as diabetes care, asthmatic care, hypertension, or other acute chronic illness, as well as testing, medication management or health maintenance counseling.
Our multi-lingual, dedicated physicians have deep experience treating the primary health issues that disproportionately affect our community including asthma, sickle cell anemia, diabetes and obesity, smoking and other addictions. By understanding the many factors affecting your health, we are better able to reduce anxiety, remove any stigma or barriers to treatment, and achieve better health outcomes for our patients.
Preventative Healthcare
We are here to help you prevent illness or detect problems before they become serious. Once you have established a primary care physician, it is important to communicate with your doctor on a regular basis. This means:
Registering for a check-up every 6-12 months
Calling your doctor after visiting a specialist or going to the emergency room
Making sure to bring all of your referral papers and other documents so that your doctor can evaluate them.

Senior Health
Better Health for Seniors
Senior Health is a specialty that focuses on the health care of older people or if the person is experiencing any combination of the following symptoms: Multiple health problems and/or Confusion or memory loss. It aims to promote health by preventing and treating diseases and disabilities in older adults. It is a multidimensional and multidisciplinary assessment designed to evaluate an older person’s functional ability, physical health, cognition and mental health, and socioenvironmental circumstances. Seniors need care and comfort to lead a healthy life without worries and anxiety. Alliance Adult Care Providers are here to take care of all your health needs.
What to Expect
Our Geriatrician trained Providers specializes in the diagnosis, treatment, and prevention of disease and disability in older adults. Geriatric physicians/providers are Primary Care doctors and mid-level, Advance Practice Nurses (APN) who are specially trained in the aging process.
Providers who focus on our health as we age, provide comprehensive medical care for people later in life. They are board certified, usually in either family medicine or internal medicine, and have received additional training and certification in geriatric medicine.
Our Providers are concerned with health problems that frequently affect older adults, such as pain, falls, memory loss, incontinence (involuntary loss of urine), and medication side effects. Another important aspect of geriatric medicine is evaluating an elderly patient’s ability to care for him- or herself (e.g., prepare and eat meals, bathe, dress).

Your Doctor/ Provider and Family Care givers work as a health care team to address issues that affect the elderly. Medical professionals who work with geriatricians include nurses, social workers, nutritionists, physical therapists, occupational therapists, pharmacists, and geriatric psychiatrists. Alliance’s Senior health care team focuses on many aspects of the patient’s life, including his or her social support (e.g., spouse, children, other relatives), living conditions and home life, and community. For the times when you need to be cared for at Home, Alliance offers Tele-Health and Home visits by our Community Support Services Team and your Doctor as needed.
